Key Clinical Studies & Reviews colds

Tomooka LT, Murphy C, Davidson TM (2000) — Clinical Study and Literature Review of Nasal Irrigation

In a prospective controlled study with 211 patients who had sinonasal disease (e.g. allergic rhinitis, post-nasal drip, etc.), pulsatile hypertonic saline irrigation (delivered via a Water-Pik device with a nasal adapter) twice daily for 3-6 weeks showed statistically significant improvement in 23 of 30 nasal symptoms. Also improved overall health status via a Quality-of-Well Being questionnaire. 

Liang KL et al. (2008) — Impact of Pulsatile Nasal Irrigation on the Prognosis of Functional Endoscopic Sinus Surgery (FESS)

This randomized controlled trial showed that in patients with mild chronic rhinosinusitis undergoing FESS, adding postoperative nasal irrigation (pulsatile / high-volume) resulted in better symptom and endoscopy score improvements compared to non-irrigation. For severe CRS the benefit was less pronounced. 

“Nasal Irrigation: An Imprecisely Defined Medical Procedure” — Principi, N., & Esposito, S. (2017)

A review that examined many studies of nasal irrigation (both acute and chronic sinonasal conditions, allergic rhinitis, prevention of upper respiratory tract infections). It finds that nasal irrigation “seems to be effective” in reducing symptoms, though points out that many studies have methodological limitations, and emphasizes the need for standardizing the methods (device type, volume, pressure, etc.). 

Effectiveness of Hypertonic Saline Irrigation following FESS — Lima AD et al. (2025)

A systematic review & meta-analysis looking at hypertonic saline irrigation in post-operative care (after sinus surgery). Found superior improvements in nasal crusting, mucosal healing, and nasal inflammatory symptoms. This supports the idea that proper irrigation (volume, solution strength) has measurable benefits in recovery. 

“A novel irrigation device with superior nasal irrigation efficiency to the classic rinse bottle” — Wu D., Chang F., et al. (2022)

This more recent study compared a novel irrigation device (extended nozzle, side opening) vs. a classic rinse bottle in 3D-printed models of the sinonasal cavity. They used simulated mucus (yogurt) and showed that the novel device had much higher irrigation efficiency (more of the simulated mucus removed), especially with adequate volume and favorable head positions. The classic bottle left more residual mucus, especially deep in regions like the olfactory cleft.
